Types of bone gouges
Bayonet septum gouges
The Killian nasal gouge and the Tilley nasal gouge are bayonet-shaped so the hand does not block the view. The Tilley tip has a V-slot that anchors on bone. Both remove bony spurs and a deflected maxillary crest in septoplasty.
Hump gouge
The Parkes hump gouge is double guarded and made for dorsal hump reduction in rhinoplasty.
Contouring gouges
The Sheehan nasal gouge and the Freer septum chisel and gouge contour nasal bone and cartilage in rhinoplasty and nasal reconstruction.
How to choose a surgical gouge
- Septal or dorsal work. Bayonet Killian and Tilley gouges reach the septum and maxillary crest; the Parkes gouge works on the dorsal hump.
- Bayonet or straight. A bayonet shank keeps the hand out of the line of sight in the nose; straight gouges suit open contouring.
- Gouge or chisel. A gouge lifts a rounded channel; a surgical chisel makes a flat forward cut — see Surgical Chisels.
What are surgical gouges used for?
Removing septal spurs and the maxillary crest in septoplasty and submucous resection, reducing the bony dorsal hump, and contouring nasal bone and cartilage in rhinoplasty and nasal reconstruction.
