Periosteal Elevators & Surgical Dissectors

BusinessMed® Periosteal Elevators & Surgical Dissectors lift periosteum, perichondrium and mucoperichondrial flaps off bone and cartilage, and separate tissue planes by blunt dissection. The range covers nasal and septum elevators for rhinoplasty and septoplasty, facial and jaw elevators, cleft palate elevators, general periosteal elevators and raspatories, and breast and gluteal dissectors.

Types of elevators and dissectors

Nasal elevators

Nasal elevators lift the periosteum from the nasal bones and the perichondrium from the nasal cartilages in rhinoplasty — Joseph, Fomon, Converse, Cottle, McKenty, Gunter and Daniel-Cakir patterns. The Boies elevator is a heavy, blunt-edged lever that lifts depressed nasal bone fragments from inside the nose.

Septum elevators

Septum elevators raise the mucoperichondrial flap from the septum in septoplasty and rhinoplasty. Double-ended patterns pair a cutting or semi-sharp end that opens the plane with a blunt end that carries the elevation on — Cottle septum, Hajek-Ballenger, Howarth. Gorney suction elevators run suction to the tip; Killian blades taper, Kistner ends are non-cutting balls, Goldman is a broad displacer.

Periosteal elevators and raspatories

The Freer elevator is double-ended, sharp and blunt, used on septoplasty, rhinoplasty and oculoplastic trays. The sharp Langenbeck starts the subperiosteal plane and the blunt one follows; the Cobb elevator strips paraspinal muscle from the laminae; the Obwegeser elevator strips periosteum from the mandible, maxilla and cranium; Each Daniel-Gubisch raspatory lifts the perichondrium around harvested rib cartilage.

Facial and zygoma elevators

Gillies, Dingman and Rowe zygoma elevators pass from a temporal incision beneath the temporal fascia to lift a depressed zygomatic arch; the Rowe has a second handle that steadies the blade.

Cleft palate elevators

Cleft palate raspatories and elevators detach the mucoperiosteal flaps from the hard palate — the right-angled Blair elevator, Cronin and Barsky pharyngeal flap patterns.

Breast and body dissectors

Breast dissectors separate the breast from the underlying fascia, or the pectoralis major from the chest wall, by blunt dissection to form the implant pocket; transaxillary dissectors are long and curved. See the Tebbetts breast dissectors, Agris-Dingman, MacCollum-Dingman, Jacobs, Reynolds and Wieslander patterns; the Gonzalez duckbill dissector for intramuscular gluteal augmentation; the Lockwood underminer for abdominoplasty; and the Mammostat breast elevator.

How to choose a surgical elevator

  • Tissue and plane. Periosteum on bone, perichondrium on cartilage, mucoperichondrium on the septum, or a blunt pocket (breast, gluteal).
  • Sharp or blunt. A sharp end opens the plane; a blunt end continues it. Double-ended patterns carry both.
  • Suction. Gorney and Haraldsson suction elevators clear the field while elevating.
  • Reach. Short nasal elevators for the nose; long curved dissectors for transaxillary and endoscopic access.

What are periosteal elevators used for?

Rhinoplasty and septoplasty, nasal fracture reduction, zygomatic arch reduction, cleft palate repair, rib cartilage harvest, maxillofacial and spinal exposure, breast augmentation, reduction and lift, gluteal augmentation and abdominoplasty.
